As an organization of caregivers, we know providing full-time care for your loved one at home can be difficult at times. That’s why we’ve developed a program at Ebenezer to give you a hand. For more than 33 years, the Ebenezer Adult Day Program has helped older adults remain as independent as possible, while giving their caregivers support and peace of mind.
We Can Help
Our warm and caring staff provides personalized care for adults 55 and older who live at home or with family, but need some assistance during the day while caregivers are at work, or simply in need of a break. Ebenezer’s adult day program also offers relief to caregivers who can rest assured that their family member’s medical, social and spiritual needs are bring met in a safe, enjoyable atmosphere.

Directions
From The South: Go north on 35W, exit on the 31st and Lake Street exit, just before downtown. Turn right on Lake Street to Park Avenue. Turn left on Park Avenue. Turn left on 25th Street. Turn left on Portland Avenue to Ebenezer Hall.
From The North: Go south on 35W, exit at the 35th and 36th Street exit, just south of downtown. Turn left on 36th Street (one way) to Park Avenue. Turn left on 25th Street. Turn left on Portland Avenue to Ebenezer Hall.
Parking
Parking is available throughout the Minneapolis campus. The parking symbol designates official parking areas. There is also on-street parking.
Meeting Your Needs
The Adult Day Program at Ebenezer Care Center is designed to meet the unique needs of adults age 55 and older, who:
- Live at home but need some health monitoring and assistance with personal care or daily activities
- Live with family and need supervision during the day while family member are at work or in need of respite
- Are in need of social stimulation, spiritual or emotional support
- Are transitioning from a hospital or care center to home
- Are in need of specialized programming due to memory or other cognitive losses
Flexible Participation
Join us once a week, twice a week or all five days. We are happy to have you participate in a way that works best for you. For more than 33 years, our adult day program has provided the flexibility to help older adults remain as independent as possible.
Nutrition
Our program fee includes a morning snack, lunch, and an afternoon snack. Together, these foods meet one-third of the minimum daily nutritional requirements established by the FDA.
Transportation
Daily transportation is available through Metro Mobility and is coordinated by the adult day program staff. Vans are air-conditioned and wheelchair accessible.
An Active Community
We believe that staying active and interacting with others is the key to an older adult’s self-worth and independence. So, we make sure the needs of each adult day program participant are met, whether they want to explore a new hobby, go on a fun, community outing or just spend a little quiet time alone. We offer a variety of recreational and social activities including:
- Exercise
- Spiritual services/worship
- Horticultural therapy program
- Cooking/baking
- Arts and crafts
- Movies/videos
- Music and singing
- Community outings
- Parties and special events
Hours and Rates
Hours
Our hours are from 9 a.m. to 3 p.m., Monday through Friday. Extended care hours are available for an additional charge.
Rates
The Ebenezer Hall Adult Day Program is very affordable and often costs less than home care services. A sliding-scale fee, as well as financial assistance, is available through Hennepin County. We accept the following methods of payment:

- Elderly Waiver
- Veteran’s Administration
- CADI
- Insurance
Basic Services:
Adult Day Program – $62.50 per day
Transportation:
Transportation is provided by the facility and is Metro Mobility certified.
Cost – $7.50 / round trip
Extended Care:
8 a.m. – 9 a.m. - $7.50
3 p.m. – 4:30 p.m. - $7.50
Personal Care Services:
Whirlpool bath (1/2 hour) - $15 / bath
Beauty / Barber Services:

Enrollment Process
Private pay individuals can self-refer to our program. Caregivers should call and schedule a meeting with the director of our program and tour of our facility. During this visit, we will determine if the adult day program can best meet the needs of your family member and, if so, will help develop a schedule for their attendance. We can also help arrange transportation at this time.
If an older adult is on county or medical assistance, a caseworker must send us an authorization of services before they can attend our program. We strongly encourage caregivers and participants to also meet with us and tour the facility before beginning our program.
When you visit us, we will ask you to complete a health assessment and application form. We require a complete medical report from a physician at the time of enrollment and yearly thereafter.
