Located just south of downtown Minneapolis, Ebenezer Care Center, with 120 private rooms and nine double rooms, is designed to look and feel more like a residence than a health care facility. Ebenezer Care Center has 34 Medicare- and Medicaid-licensed and certified skilled nursing beds, allowing opportunity to enjoy a private room. Our residents also have secure access to the Ebenezer Care Center Healing Garden, a private therapeutic garden overseen by a certified horticultural therapist. A full-time social worker and full-time chaplain are also available to residents.

From the south -- Go north on 35W, exit on the 31st and Lake Street exit, just before downtown. Turn right on Lake Street to Park Avenue. Turn left on Park Avenue. Turn left on 25th Street. Turn left on Portland Avenue to Ebenezer Care Center.
From the north -- Go south on 35W, exit at the 35th and 36th Street exit, just south of downtown. Turn left on 36th Street (one way) to Park Avenue. Turn left on 25th Street. Turn left on Portland Avenue to Ebenezer Care Center.

Skilled Nursing Services
Ebenezer Care Center has 34 Medicare- and Medicaid-licensed and certified skilled nursing beds. This option provides skilled care for residents who need round-the-clock assistance and medical attention. Long-term care services include but are not limited to:

Board and Care
Ebenezer Care Center has 107 Medicaid licensed and certified board and care beds. Board and care is for individuals who are able to live independently but need some supervision and personal care assistance. Our board and care residents are active inside and outside the facility and come and go as they please. But daily contact and support from our staff ensures that they are safe and that any health care needs that may arise are met.
Basic board and care services include:
- Private/semi-private room with emergency response system
- Three meals and snacks per day
- Help with daily activities such as money management and scheduling health care appointments
- Medication administration
- Laundry and housekeeping
- Social, recreational and spiritual activities
More traditional long-term care services, such as bathing assistance, medical treatments and 24-hour licensed nursing care, are also available for board and care residents if and when they are needed.
Memory Care
Our memory care unit of 25 private rooms meets the special needs of those in early- to mid-stage dementia. Our staff members on this secure unit are specially trained, ensuring that your loved one receives the special care and attention he or she needs. Residents can participate in daily small group or in one-on-one activities. Whenever possible, memory care residents may participate in Ebenezer Care Center activities, such as religious services and special meals and events.
The memory care unit includes a common area with a working oven so residents can participate in baking activities. Other familiar daily activities include washing dishes, folding clothes and sweeping. And, memory care residents are invited to visit our secure Healing Garden.
Deaf Services
Ebenezer Care Center has a special unit designed to meet the needs of deaf residents:
- American Sign Language interpreter on staff
- TTY and other communication equipment
- Trained staff

Adult Day Program
The Ebenezer Adult Day Program, a daytime community for older adults, is located on the lower level of our building. Our warm and caring staff provides personalized care for adults 55 and older who live at home or with family, but need some assistance and socializing during the day while caregivers are at work or in need of respite. Meals Made to Order
The meal program at Ebenezer Care Center is designed to meet the special requests of our residents. Using special computer software, we track the likes and dislikes of all our residents, as well as their food allergies, and then use this information to develop customized meals for our residents.
An Active Community
We make sure the needs of each resident at Ebenezer Care Center are met, whether they want to explore a new hobby, go on a community outing or just spend a little quiet time alone. We have on-site certified therapeutic recreation specialists, a certified music therapist, horticulture therapist and ordained chaplains who offer a variety of recreational, social and spiritual activities for all our residents including:

Admission referrals
The goal of our admissions team is to provide the best customer service possible to our referral sources. To make the admission process as quick and easy as possible, Ebenezer Care Center admits new residents 24-hours-a-day, seven-days-a-week. Admission phones are staffed 8 a.m. to 4:30 p.m., Monday thorough Friday, with a nursing supervisor available after hours and on weekends, evenings and holidays. A nurse assessment will be conducted.
Because of our flexible admission hours, referral sources can start the admission process late in the afternoon without having to wait until the next day to get their patients placed. Weekend admissions do not have to be pre-planned because staff is always available to take your call on weekends, evenings and holidays.
