Meadows on Fairview offers assisted living and memory care in a beautifully appointed prairie-style building in the tranquil countryside on the Fairview Lakes Campus in Wyoming, Minnesota.

Meadows provides the perfect apartment home setting for seniors needing daily assistance or for those with mild to moderate memory loss. We also offer outstanding services designed to promote wellness and independence.

Meadows on Fairview is owned and managed by Ebenezer, a trusted name in senior services since 1917. Ebenezer is a part of Fairview Health Services.

Make Yourself at Home

Our community features comfortable gathering spots to read a book or visit with friends and family. Residents are always welcome to use the meditation room, private dining room, beauty and barber salon, fireside parlor, and crafts room. These well-appointed common spaces provide many opportunities for residents to meet new friends, try new activities and participate in our leisure and educational programs.

The out-of-doors is just a step away as residents may enjoy our private patio, screened-in porches and enclosed memory care garden. And wooded walking paths are adjacent to the building on the Fairview Lakes campus.

Each of Meadows’ floor plans is designed for comfort and convenience with kitchens, private baths, lots of closets, and lovely views of the prairie or the stately pines.

Your assisted living monthly fee includes the following services:

• Weekly light housekeeping
• Weekly flat linen laundry service
• Three restaurant-style meals daily plus snacks
• All utilities (except phone) and basic cable television
• Emergency call pendant
• 24-hour personal care staff
• Medical transportation arrangements
• Wellness programs, blood pressure clinics and health care observations
• Leisure activities and educational programs

As your needs change, we can tailor a Meadows’ service package to best accommodate you each day.

Whether you prefer “a la carte” services like a trip to our beauty salon, or “Spring” cleaning for your apartment, or you need assistance getting started each morning, Meadows’ staff is there when you need them.

Meadows’ personal care services, offered at an additional charge, include: dressing in the morning and/or for bed, personal grooming, oral care, hair care, toilet use, medication monitoring and reminders, diabetes assistance, bathing assistance, and help with laundry.

Services vary per plan. Each plan is based on an individualized assessment and service plan. The total cost for the plan is based on a point system. Points are based on the type of service and the frequency with which the service is provided.

At Meadows, you can choose from 18 unique apartment sytles ranging from studios to two-bedrooms.

Assisted Living Apartments
Meadows’ assisted living apartments have kitchens with full-size appliances and built-in microwaves. Select units have dens with French doors; decks or patios; and two bathrooms. Pick your choice!

• Studio -- 355 sq ft.
• One-bedroom -- 529-614 sq. ft.
• One Bedroom + den -- 802-924 sq. ft.
• Two-Bedroom -- 1,018-1,169 sq. ft.

Memory Care Apartments
All of Meadows’ spacious memory care studios have kitchens, private bathrooms and large closets to help make residents feel right at home.
